   QABC Senior Scholarship 

The QABC will award a $1,000 scholarship each to a male and female student/athlete who have been active in OPHS athletics and have been a participant in at least 2 varsity sports.

 

All athletes must fill out the QABC scholarship application below and include 2 OPHS coach recommendations.

SCHOOL ATHLETIC EXPERIENCES: List all school athletic teams in which you have participated during the last 4 years (JV and Varsity). Indicate all special awards and honors. List all leadership positions.  Check the academic years during which you participated

COACH RECOMMENDATION INSTRUCTIONS: Two coach recommendations are required to be considered for the scholarship award. Students - please email your coach the message below.

I am asking you to be a reference for me for the QABC 2024-2025 scholarship. This $1,000 scholarship is given annually to one male and one female athlete who has been active in the OPHS athletic program and has been a participant in at least two varsity sports.


Please share with the scholarship committee how I demonstrate some or all of the qualities that this award represents: positive attitude, work ethic, commitment, leadership, reliability, punctuality, self-motivation, team player, and resourcefulness.  


All letters should be emailed to the QABC Scholarship committee at quakerathleticboosterclub@gmail.com with the subject: QABC Scholarship Letter of Recommendation and my last name. You will receive an email confirmation that your letter was received.  Your letter of recommendation will only be seen by the committee members.
